Patient Event Entry Portal
The Patient Event Entry Portal enables our partnering post-acute facilities to check their admitted patient’s ACO eligibility. When patients are matched, facilities can generate an ADT alert which will be sent to the patient’s primary care physician through the SuperDocACO App.
